Important Announcement Regarding Internship – II !
Dear Students,
The internship insurance required for the internship period will be provided by the university.
Before starting the internship, students must receive Occupational Health and Safety training and document that this training has been successfully completed.
All documents within the scope of Internship II should be obtained in PDF format from the internship section in the student menu of the KOSTÜ Pharmacy website and used accordingly.
Students must fill out the Compulsory Internship Form within the scope of Internship II and submit it to the Dean’s Office with a wet signature and stamp, along with a photocopy of their identity card, by 15:00 on May 15, 2026.
Students will submit the Internship Evaluation Form and the General Information Form to the authorized person at the institution where they will be doing their internship.
Students will fill out the Internship Logbook in accordance with their learning objectives. The internship logbook must be filled out legibly with a blue ballpoint pen and stamped and signed by the authorized institution official. (Pencil use is prohibited.) The signature and stamp of the authorized institution official are required on all pages, including the pages where the report is written. Logbooks that do not comply with the procedure will not be accepted.
Internship logbooks will be obtained ready-made from the contracted stationery store.
It should be remembered that the internship logbook is a scientific report and should be written accordingly.
Students in the English program will write the pages in their logbooks in English.
The Internship Evaluation Form will be signed and approved by the authorized pharmacist after the internship is completed and given to the student in a sealed envelope (the part where the envelope is sealed must be wet-signed). Unapproved forms will not be considered.
The student must submit the internship logbook and the internship evaluation form to the Internship and Professional Practice Commission within 15 days of the start of the 2026-2027 Fall semester.
Students cannot change their internship location after starting their internship. In the event of death, natural disasters, or extraordinary circumstances that make it difficult for the pharmacist/student to continue, the internship location may be changed with the recommendation of the commission and the approval of the Dean’s Office.
Students are required to comply with the provisions of the Higher Education Institutions Student Disciplinary Regulations, the working principles of the institution where they are interning, and the rules regarding working conditions, discipline, and occupational safety during their internship.
If a student is found to be absent from their internship location without a valid excuse during inspections conducted by the Internship Commission, their semester internship will be canceled, and the student will fail this internship.
The student will complete this internship in a public pharmacy or hospital pharmacy within the borders of Turkey, at their own discretion. The pharmacist in charge of this private pharmacy must have at least 5 years of experience as a private pharmacy manager.
*For students who will complete Internship II in a private pharmacy:
They must contact the pharmacist of the private pharmacy where they wish to intern to arrange the pharmacy themselves. Internship II will be 20 working days. For independent pharmacies, Saturdays should be counted as working days in the internship, as pharmacies open on Saturdays are considered working days. For pharmacies closed on Saturdays, working days should be calculated accordingly. (The periods for internships in independent pharmacies are specified below.)
*For students wishing to complete Internship II in a private hospital within or outside the Kocaeli province:
Students must contact the private hospitals where they wish to intern and submit their internship applications independently. Saturdays are not working days in hospitals, therefore they are not included in the internship. (The periods for internships in hospital pharmacies are specified below.)
*For students who will complete Internship II in a hospital affiliated with the Ministry of Health within the Kocaeli province:
They must apply to the Presidential National Internship Program.
For students who will complete Internship II in a hospital affiliated with the Ministry of Health outside the Kocaeli province:
They must apply to the Presidential National Internship Program.
Those who have not applied to this program should contact the hospitals themselves and follow the hospital’s instructions.
The student submits the Internship Evaluation Form and the General Information Form to the authorized person at the institution where they will be doing their internship. (These forms can be accessed from the faculty website.)
Internship II will be conducted over 20 working days, with a minimum of 6 hours per working day, for a total of at least 120 hours. In pharmacies open on Saturdays, Saturdays should be counted as working days. In pharmacies closed on Saturdays, working days should be calculated accordingly. Saturdays are not counted as working days in hospital internships.
In the evaluation of Internship II, the internship logbook and the internship evaluation form are taken into consideration, and the internship exam will be announced and conducted in the Fall semester. The average of the report and the exam score, provided a minimum of 70 points is obtained on the exam, will be used to evaluate Internship II as Pass/Fail.
